Orange County Detox Center is committed to protecting the privacy of the personal and health information we collect or create as part of providing health care services to our clients, known as “Protected Health Information” or “PHI.” PHI typically includes your name, address, date of birth, billing arrangements, care, and other information that relates to your health, health care provided to you, or payment for the health care provided to you. PHI does not include information that is de-identified or cannot be linked to you.

This Notice of Health Information Privacy Practices (the “Notice”) describes Orange County Detox Center’s duties regarding the privacy of PHI, our use and disclosure of PHI, client rights, and contact information for questions, comments, or complaints.

ORANGE COUNTY DETOX CENTER’S PRIVACY PROCEDURES AND LEGAL OBLIGATIONS

We obtain most PHI directly from you through applications, assessments, and conversations. Additional information may be collected depending on your needs and your consent to make referrals or inquiries. We may also receive PHI from health care providers, government agencies, or community health organizations to coordinate services.

We are required by law to provide this notice and to follow the terms stated within. Orange County Detox Center reserves the right to revise this notice at any time, and any revisions will apply to all PHI we maintain, including information collected prior to changes. Revised notices will be made available upon request or mailed to you directly.

We are legally obligated to protect PHI and comply with all applicable federal and state privacy laws. Our staff follows strict policies and procedures to ensure your information is handled securely and respectfully.

USES AND DISCLOSURES WITH YOUR AUTHORIZATION

Except where outlined below, Orange County Detox Center will not use or disclose your PHI without your written authorization. You may revoke this authorization at any time, except when we’ve already taken action based on it. Authorization forms are available upon request.

PERMITTED USES AND DISCLOSURES WITHOUT AUTHORIZATION

For Treatment:
We may share PHI with physicians, therapists, nurses, and other healthcare professionals directly involved in your care.

For Payment:
PHI may be used or shared with insurers, government agencies, or health plans to obtain payment for services rendered. For example, we may provide dates of service to your insurance provider.

For Health Care Operations:
We may use PHI to support internal operations like quality assurance or administrative functions. PHI may also be shared with business associates that help us perform essential tasks. These partners are required to maintain strict confidentiality.

As Required by Law:
We may disclose PHI when mandated by law, including for court orders, government agency requests, or reports of abuse, neglect, or domestic violence.

For Public Health Activities:
We may release PHI to public health authorities as required, such as for communicable disease reporting.

For Health Oversight Activities:
PHI may be disclosed to oversight agencies for audits, inspections, licensure, or investigations.

To Prevent Serious Threats to Health and Safety:
We may disclose PHI to prevent or lessen a serious and imminent threat to a person’s or the public’s health or safety.

Health-Related Services and Benefits:
We may contact you with information about services, treatments, or resources that may benefit your health or recovery journey.

Incidental Disclosures:
Minimal disclosures that occur as a byproduct of permitted uses (e.g., overheard conversations) are allowed if we use appropriate safeguards.

To Personal Representatives:
We may disclose PHI to a person legally authorized to make health care decisions on your behalf, according to state law and documentation on file.

To Family and Friends:
We may share limited PHI with family or close contacts involved in your care or payment, if you allow it. In emergencies or if you’re unable to agree, we may determine what is in your best interest and share accordingly. You may limit or prevent such disclosures.

Disaster Relief:
We may share information with disaster relief organizations to help locate and inform family members or caretakers.
